Instructions:

 

Box 1: The ARES Group name this report is for.

 

Box 2: Month the report is covering.

 

Box 3. Input the total number of ARES volunteers for this group.

 

Box 4. The total number of ARES nets conducted for the reporting month. Do not include training nets. Those are included in box 6a.

 

Box 5. How many of the nets from Box 4 have a NTS Liason.

 

Designate the county from which your group resides.

 

Box 6a: Input the total number of exercises and training sessions. This does not include regular nets unless that net is a training session.

 

Box 6b: Person hours is the total time all individuals were participating. For example if your training (net or training meeting) was 2 hours, and there were 10 participants, the Person hours reported would be 20.

 

Box 7a: The number of community events supported such as parades, fun runs, etc.

 

Box 7b: Total number of person hours for the event.

 

Box 8a: The number of emergency operations/ARES activations.

 

Box 8b: Total number of person hours for the operation/activation.

 

Box 9a: Number of Skywarn operations/activations.

 

Box 9b: Total number of person hours for the operation/activation.

 

Boxes 10a and 10b are automatically totaled.

 

Box 11: Any comments or clarifications for the report, upcoming events, etc.

 

The name of person completing the report along with their callsign and email address.
